DESCRIPTION:Collaborative Learning Student Series – Electives \nThe Collaborative Learning Student Series Electives will take place on various days and times throughout the Spring and Summer. Many of the activities have limited registration and learners are encouraged to sign up early to secure a spot. Registration will be open 2 weeks prior to the event. \nProgram: Stroke Rehabilitation Care \nObjectives: \n1. Appreciate various team members’ roles in developing successful discharge planning back to community\n2. Describe the realities of available treatment time\n3. Describe the community/discharge resources available for patients who have suffered a stroke \nRegistration:  \nClick this link to register \nAccreditation:\nThese learning activities are approved by the University of Toronto\, Centre for Advancing Collaborative Healthcare & Education (CACHE) for IPE credit and most will take place over Zoom.

Learners are expected to participate in all 4 sessions.
